Pharmacist

This role focuses on advancing evidence-based, cost-effective medication management for patients with kidney and cardiac conditions and related comorbidities. You will work at the intersection of clinical pharmacy, population health, and care management to optimize medication regimens and improve patient outcomes.

The position involves addressing medication access, affordability, education, adherence, and safety while supporting transitions of care and chronic disease management. You will collaborate closely with physicians, pharmacists, care teams, and other healthcare professionals to identify opportunities for improved medication use.

The role also contributes to pharmacy strategy, clinical programs, treatment guidelines, and quality improvement initiatives. This is a fully remote opportunity for an experienced clinical pharmacist with strong analytical, communication, and strategic skills.
